The Clinical and Statistical Study on Breech Delivery


Abstract
The breech delivery is the most frequent malpresentation with high perinatal mortality experienced by the obstetricians, nowadays. The results of the statistical study of 198 cases of breech deliveries in the department of Obstetrics, Korea University Medical College Hospital from January 1^(st), 1966 to December 31^(st), 1970, are as follows:
1) The frequency of breech delivery was 4.6 percent. (198 breech deliveries among 4284 total deliveries)
2) The primipara of breech delivery was 52% and that of the multipara was 48% among total breech deliveries, and the most of them was in between 25 to 29 years of age.
3) The premature delivery was 285(6.5%) among 4284 total deliveries while there were 59(30%) among total breech deliveries and the frequency of breech delivery reduced as the total body weight increases.
4) The incidence of breech delivery was high at the early gestational week. The incidence before 24th week of gestational period was 25% while 3.3% between 37th and 40th week of gestational period.
The spontaneous rotation occurs in almost cases of malpresentation as the gestational period gets older
5) The most frequent type of breech delivery was the frank breech (30%).
The frank and complete breech presentaion were higher in primipara than multipara,¢¥and the incomplete breech presentation were higher in multipara than primipara.
6) The most frequent indication of the total breech extraction was the labor arrest(18 cases) and the perinatal mortality was 19%.
7) The incidence of the Cesarean section was 22(110). There was one perinatal death by the combined congenital cleft palate.
8) The most frequent indication for Cesarean section was feto-pelvic disproportion.
9) The corrected perinatal mortality of the breech delivery was 10. 1% in premature and 3.6% in mature deliveries.
